Our Mission

Our mission is to provide our clients with caring companionship, compassionate personal care assistance, and conscientious nursing services.

Realizing an increased need for providers to serve the elderly and diversely-able populations, Transitional Independence, LLC was created with a vision of providing comprehensive care services to meet the unique needs of seniors and individuals with disabilities in our community.

Our Leadership

Melanie Phillips Brown

Administrator
Melanie Phillips Brown, born and raised in Columbus, GA, has a passion for helping others and providing quality service.

She is an alumna of Columbus State University graduating with her Bachelor’s, Master’s, and Specialist’s degrees.

Melanie has 16 years of professional experience working as a school counselor in Stewart and Muscogee County School Districts. She has a degree in Computer Science, School Counseling, Educational Leadership, and is a licensed insurance agent.

Melanie is also a wife and mother of two.

Our Caregivers

CREDENTIALS

As required by the Georgia Healthcare Facility Regulation Division:

Our companion/sitters will meet minimal education requirements, possessing a high school diploma or GED. If requested, we will provide a licensed CNA.

Our Personal Care Assistants (PCA) will be verified Georgia licensed Certified Nurse Aides (CNA).

Our nursing services will be provided by a certified Georgia Licensed Practical Nurse (LPN) and/or a licensed Georgia Registered Nurse (RN).

SCREENING PROCESS

All caregivers will be required to pass a criminal background check that includes state and FBI fingerprinting through the Georgia Criminal Background Check System.

We will also check professional and personal references for each of our caregivers.
